Metabolic Bone Disease in Reptiles: The Calcium Failure You Can Prevent

I watched a young bearded dragon die of metabolic bone disease once, and the worst part was how preventable it was. The owner had done everything right except the two things that matter most: the UVB bulb had burned out three months earlier, and the calcium supplement sat untouched in the cabinet because the lizard "seemed fine." Seemed fine. Right up until its jaw went soft and its legs started trembling.
MBD isn't a rare exotic disease you'll probably never see. It's the single most common health problem in captive reptiles — the one exotic vets see weekly, in bearded dragons, leopard geckos, iguanas, turtles, chameleons, and pretty much anything with a spine that lives in a terrarium. The good news: it's almost entirely preventable with three things you control directly. The bad news: most owners don't know what those three things are until it's too late.
What MBD Actually Is
Metabolic bone disease is a catch-all term for a group of calcium metabolism failures. The most common form in pet reptiles is nutritional secondary hyperparathyroidism — which is a mouthful that just means: the body is pulling calcium out of the bones because there isn't enough available in the blood.
Here's how it works. Calcium is essential for muscle function, nerve signaling, and blood clotting — so essential that the body will cannibalize its own skeleton to keep blood calcium levels stable. When a reptile can't get enough calcium from its diet, or can't process it because it's missing vitamin D3, the parathyroid gland cranks out hormones that dissolve bone to release calcium into the bloodstream. Over weeks and months, the skeleton softens. The jaw gets rubbery. The spine curves. The legs bow. The bones fracture under the reptile's own body weight.
The Association of Reptilian and Amphibian Veterinarians describes MBD as the most commonly diagnosed nutritional disease in captive reptiles, and the cause is almost always the same triad of failures: inadequate UVB lighting, inadequate calcium supplementation, or both. Sometimes it's one. Often it's both. Occasionally it's a third factor — no D3 in the supplement, or a diet heavy in foods that bind calcium (like spinach and oxalate-heavy greens fed as a staple).
Why UVB Is Non-Negotiable
Reptiles can't use dietary calcium without vitamin D3, and most reptiles can't make vitamin D3 without UVB light. The chain goes: UVB hits the skin → the body synthesizes vitamin D3 → D3 enables calcium absorption from the gut. Break any link and calcium absorption collapses, no matter how much calcium you're dusting onto the food.
This is why "I give calcium powder every feeding" isn't enough by itself. If the UVB is missing, most of that calcium passes right through. And here's the trap: UVB doesn't pass through glass or plastic. A reptile sitting by a sunny window is getting heat and visible light but essentially zero UVB — the window filters out the wavelengths that matter. Each species needs a specific UVB strength and distance. A bearded dragon needs a much stronger setup than a leopard gecko (which is crepuscular and needs far less, though still some). Calcium: The Dusting Math
The other half of the equation is dietary calcium. In the wild, reptiles eat whole prey — bones, shells, and all — which delivers calcium in the right ratio to phosphorus. In captivity, most reptiles eat gut-loaded insects, vegetables, or thawed rodents, and those foods are calcium-poor and relatively phosphorus-rich. A cricket has a calcium-to-phosphorus ratio around 1:7, which is backwards from what a reptile needs (roughly 2:1).
The fix is dusting: coating feeder insects in a calcium powder before feeding. The standard protocol for most growing reptiles is calcium with D3 at most feedings, and a multivitamin (which includes D3 and other fat-soluble vitamins) once or twice a week. Gut-loading means feeding the insects a nutritious diet for 24-48 hours before they go in the reptile's tank. Crickets and roaches that ate calcium-rich gut-load food become calcium-rich themselves — the calcium goes into their gut and body tissue, and the reptile gets it with the meal. Commercial gut-load diets exist, but you can also use dark leafy greens, squash, and sweet potato. The Species Most at Risk
MBD is not democratic — some reptiles are far more prone to it than others. The highest-risk pets in the hobby:
- Bearded dragons — fast-growing, high calcium demand, and routinely kept with inadequate UVB. The classic MBD poster child.
- Iguanas — herbivores that need massive calcium intake and strong UVB. They also grow fast, which compounds the demand.
- Veiled chameleons — notoriously difficult to supplement correctly because they eat mostly insects and have specific UVB needs.
- Turtles and tortoises — shell growth demands enormous calcium. A soft shell is an MBD red flag.

The Warning Signs: What MBD Looks Like
MBD is insidious because it develops over months, and reptiles hide illness the way prey animals do. By the time you notice, the bone damage is often already done — but catching it early limits the permanent damage and can still save the animal. The signs, roughly in order of appearance:
- Lethargy and reduced appetite — the first, vaguest signs. The reptile is less active, less interested in food.
- Tremors or twitching — especially in the toes and legs. This is the neuromuscular sign of low blood calcium, and it's a genuine emergency. A reptile with tremors needs a vet now.
- Soft or rubbery jaw — press gently on the lower jaw and it flexes instead of being firm. This is the "rubber jaw" that's the classic MBD finding.
- Bowed or swollen limbs — legs that curve, or joints that look swollen or lumpy.
- Spinal kinks or a hunched back — the spine deforming under the body's weight.
- Difficulty walking or climbing — dragging limbs, struggling to grip, falling off branches.
- In turtles: a soft, deformed shell — a shell that depresses under light finger pressure is an emergency.
- Fractures — bones breaking from normal activity because they've become brittle.
Any one of these warrants a vet visit. Tremors, soft jaw, and fractures are emergencies — don't wait for a scheduled appointment. MBD caught early can often be reversed with aggressive correction of UVB, calcium, and diet. MBD caught late leaves permanent deformities and, in severe cases, is fatal.
What the Vet Will Do
Diagnosis usually starts with a physical exam and a history — the vet will ask about your UVB bulb (type, age, distance), your supplement routine, and your diet. X-rays are common: they can show reduced bone density, deformities, and old fractures. Blood work can measure calcium and phosphorus levels.
Treatment depends on severity. Mild cases: correct the UVB, fix the supplement schedule, adjust the diet, and monitor. Moderate to severe cases: injectable or oral calcium, vitamin D3 therapy, sometimes hospitalization with intensive nursing care, and pain management for fractures. Deformed limbs may never straighten, even after treatment. That's the permanent damage part — and it's why prevention beats everything.
Prevention: The Three-Part Checklist
MBD prevention is a checklist, not a mystery. Run through it monthly:
- UVB: Is the bulb the right type for your species? Is it a linear tube (or another genuinely effective source), not a window or a screw-in "daylight" bulb? Is it the right distance from the basking spot — not too far to deliver effective output? When did you last replace it? (Most tubes: 6-12 months. Write the date on the tube with a marker.)
- Calcium: Are you dusting feeder insects or food at the right frequency for your species and age? Are you using a calcium powder with D3 (for most species), stored properly, and not expired? Are you using a multivitamin on the right schedule?
- Diet: Are you gut-loading insects? Is the plant matter appropriate — not a staple of oxalate-heavy greens like spinach, which bind calcium? Is the calcium-to-phosphorus balance of the overall diet reasonable?

One More Thing: The UVB Bulb Calendar
Every MBD case I've seen has one thing in common: the owner couldn't say when the UVB bulb was last replaced. Not one. They could tell me the brand, the wattage, even the day they brought the reptile home — but the bulb replacement date was always a shrug.
Fix that right now. Take a permanent marker and write today's date on the end of your current UVB tube. When you replace it, mark the new date on the new tube. Put a reminder in your phone for the replacement date based on your bulb type. UVB bulbs don't announce when they've stopped working — they just quietly stop doing the one job they exist to do.
That marker stroke is worth more than any supplement you'll ever buy. It's the difference between a reptile with strong bones that lives a full lifespan and a reptile with a bent spine that dies at two years old with its owner saying "I don't understand what happened."
